Qualification  Requirements

 

• Must be uninsured I underinsured (with household budget info)  and  a  Stark County resident
• Must be at or below 250°/o  of Federal Poverty  Guidelines:

 

One Person Household   =	$ 29,425
Two Person Household   =	$ 39,825
Three Person Household =	$ 50,225
Four Person Household  =	$ 60,625

 

Must bring for an appointment

• Income Verification for all household members

     *At least 2 pay stubs for each member & Prior year tax return if filed

• Photo Identification & Verification of address if not the same as what is on the Photo ID
• Social Security Card
• Bring prescriptions, medicine bottles (must have current scripts or refills available)
